About Fenix Tactical Healthcare

The Fenix name was founded in 2017 with the creation of Fenix Physical Therapy, born from a vision to treat firefighters like professional athletes—restoring not only their function but their ability to perform under real-world demands. From the beginning, Fenix was built around one principle: rehabilitation should not stop at daily living—it must restore performance.

As our experience deepened within the fire service, we saw firsthand the inefficiencies of a broken healthcare system. Firefighters were being pulled off the truck unnecessarily. Needed treatment was delayed by paperwork, referrals, and generalized therapy models that didn't understand the tactical nature of their work. We set out to fix that.

To eliminate these barriers, we established Fenix Medical—streamlining the referral and documentation process so first responders could see qualified clinicians quickly, receive tailored care, and return to duty faster. This innovation saved municipalities hundreds of thousands of dollars in lost work time while improving the quality and speed of care.

But physical injuries were only part of the picture. Working alongside firefighters, police officers, and tactical professionals revealed that not all injuries are visible. The weight of trauma, stress, and operational pressure often took as much of a toll mentally as physically. This realization led to the formation of Fenix Mental Health, completing the continuum of care.

Together, Fenix Physical Therapy, Fenix Medical, and Fenix Mental Health form Fenix Tactical Healthcare—an integrated system built out of necessity to serve the men and women who must perform at their best when others are experiencing their worst.

Defining Tactical Healthcare

Tactical Healthcare is an integrated model of medical, physical, and psychological care designed to optimize readiness, performance, and recovery for individuals operating in high-demand or high-risk professions—such as first responders, military personnel, and tactical athletes—through proactive, functional, and mission-specific healthcare interventions.

This model bridges:

  • Medical care: diagnosis, treatment, and rehabilitation
  • Performance training: strength, endurance, and mobility optimization
  • Occupational readiness: job-specific resilience and duty requirements

It moves beyond symptom management to long-term functional optimization, preparing individuals to perform safely and sustainably under stress, fatigue, and real-world operational demands.

The Fenix Performance Philosophy

At Fenix Tactical Healthcare, rehabilitation is performance. We believe that simply restoring movement is not enough—our goal is to return every patient to a level of function and performance that exceeds pre-injury capacity.

Our clinicians approach every case with a performance mindset. Whether we're treating a firefighter, police officer, or competitive athlete, we don't stop at helping them perform basic activities of daily living—we restore and strengthen them for the demands of their mission, their sport, or their career.

Equally important is prevention. If we fail to prevent re-injury after rehabilitation, we have not fully done our job. Every plan of care integrates injury prevention, resilience training, and performance conditioning to ensure durability, longevity, and confidence in every movement.

This commitment to performance-based rehabilitation defines who we are—and why the Fenix name continues to rise as the new standard for Tactical Healthcare.

Fenix Tactical Healthcare specializes in first responder work related injuries. From your initial injury, to your rehab, and getting you back on the truck or the street, we'll help you navigate the process.

Hi, my name is Austin Fair, Co-Owner of Fenix Healthcare. I believe our life experiences prepare us to help others. At a young age, I was active as any normal kid. But I was blessed with a unique talent--to injure myself. Through my formative years, I experienced injury after injury. I felt a lot of frustration. I experienced successful recoveries just in time to injure another body part, or the same body part again. I thought, "how could someone so fit and active get injured so often?" Some people had answers for me while others didn't. I decided to be responsible for my own rehabilitation and to learn everything I could to fix myself. After I helped myself, it was time to share my knowledge and experience with others to help them avoid unnecessary pitfalls and roadblocks in returning to optimal performance.

My name is Brad Hall, Physical Therapist and Co-Owner of Fenix Healthcare. I am dedicated to rehabilitating and building strong relationships with my patients. Growing up in Tempe with four siblings in various healthcare fields, I've always been inspired to help others. Among our family of physicians, dentists, and nurses, I found my calling in physical therapy where I can connect deeply with my patients on all aspects of health. My family likes to joke, we can help anyone, "from toes to teeth".
Driven by a love for people, I prioritize being there for my patients, believing that true healing involves the heart and mind as much as the body. Our one-on-one business model leverages extensive experience to provide personalized care. My extensive experience, dedication and personal touch set me apart, making me a trusted partner in your journey to better health and well being. Hi, my name is Maddy Bohne. From a young age, I knew I wanted to be a physical therapist. I walked into a clinic and fell in love with the concept of physically helping someone be the absolute best version of themselves. I also fell in love with the beauty of the human body and its ability to change and recover. I am blessed to be able to be passionate about what I do every day. I believe the acquisition of knowledge to better yourself and those around you is important for success and happiness.
After graduating from PT school, I have had the opportunity to treat patients in a variety of settings, from the hospital to the outpatient clinic. I did this to build my skill set and best treat all of my patients. I love working with all age groups, but have a special passion for sports enthusiasts and first responders. I am determined to find the cause of all injuries so that it does not reoccur. My top priority is assisting patients on their journey to full recovery.

Hi, my name is Isaiah Mews, and I'm an injury prevention and performance specialist passionate about preparing people for the demands of both life and duty. What attracted me to this field of study is the God given ability for our bodies to adapt to the stresses that are placed on them. This can be leveraged to create stronger, more resilient, and fruitful stewards of their bodies.
I've had the privilege of working across a wide spectrum: from tactical populations like fire and police departments to NCAA athletes competing at the highest level. Along the way, I've seen that readiness isn't just about lifting heavier or running faster; it's about moving better, recovering fully, and being resilient when the stakes are high.
Through years of hands-on coaching, research, and collaboration with tactical and athletic professionals, I've developed a commitment to evidence-based performance and treatment. My goal is simple: to help others become great stewards of movement, avoid unnecessary setbacks, and perform at their best when it matters most.
